New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. All of the necessary forms can be collected from reception, or if you prefer, you can download the forms from this website (see registration packs tab above) complete them and bring them to the surgery with your ID.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
From the 1st April 2015 practices are required, under the GMS contract, to allocate a named accountable GP to all patients including children This named accountable GP is responsible for the patient's overall care at the Practice.
Medical treatment is available from the date of registration. Please contact reception for further information.
Accessible Information Standard
The Accessible Information Standard aims to make sure that disabled people have access to information that they can understand and any communication support they might need.
Should you have any requirements or know of a patient who needs support please let the Practice know or complete the accessible information form that can be found on the registration packs page and we will help provide support, whether that is by providing information in large print or putting a patient in touch with British Sign Language (BSL), an interpreter, email or braille.
